 - Full name: must be exactly as it is indicated on your passport
 - Passport number
 - Date of birth
 - Gender
 - Passport expiration date
 - Nationality

Please remind that the personal information in the visa application form must be exactly shown as in your passport - You don’t need to provide the exact date of arrival but it should be the best estimated arrival date - The proposed date of arrival you entered in the form will be the issuance date of visa and your visa will be valid from this date - You are allowed to enter and exit Vietnam anytime during your visa validity.

In addition, it is fine if you change your arrival airport at last minute - The Vietnam Immigration Department does not mind at all when your real aririval airport is not what you submitted in the visa application.

One more thing, the purpose of visit is actually not very important - Nothing happens if you apply for a Vietnam tourist visa to conduct business or  apply for a business visa to visit your family.
